The film picks up immediately after the end of the last film, with Marybeth Dunston (Danielle Harris) blowing off the head of Victor Crowley (Kane Hodder) with a shotgun. She walks away and comes across the bodies of Vernon and John, where she hears Crowley in the woods. Marybeth starts up the chainsaw, but is grabbed by Victor. Marybeth kicks him, and he falls backwards onto the chainsaw, cutting him down the middle. She grabs a shotgun from one of the men and shoots Victor's head, and walks back into the city. She walks into the Jefferson Parish Police Department, immediately having guns drawn on her. Upon discovery of the twenty to thirty bodies at Honey Island Swamp, she is placed as the prime suspect in the murders by Sheriff Fowler (Zach Galligan), even after telling him numerous times about the events of the last two films. The Sheriff heads out to the swamp with the paramedics and fire department, leaving Deputy Winslow (Robert Diago DoQui) in charge of the station until he gets back.

The basic storyline of the film follows Bobby and Scott, two young men in their late twenties who are struggling to make careers out of their artistic dreams but their competitiveness with each other gets in the way. Bobby is a screenwriter and Scott is a musician but neither of them are productive because they don't believe in themselves and they share incredibly high standards for their work. Bobby meets an old lover, Deirdre, who is headed to Los Angeles and she begs Bobby to take care of her dog while she's gone. Bobby agrees but only in exchange for a sexual favor. Later, when Bobby makes the mistake of sharing an idea for the opening of his new screenplay with Scott, Scott tells their mutual friend, Hart, a playwright, and Hart steals the idea, incorporating it in his new play "Death of a Banker", a campy morality tale about the September 11, 2001 attacks set to be performed off-Broadway later that month. Bobby loses Deirdre's dog and tries to figure out a way to get out of the situation.

Physics professor Zach Shefford (Grayson McCouch) has regarded his telekinetic gifts as a curse rather than a blessing. This sentiment is obviously not shared by ruthless Pentagon agent Raymond Addison (Louis Gossett Jr.), who recruits Shefford for a dangerous mission in which his "second sight" talents will be taxed to the utmost. It seems that, back in 1977, Addison had overseen Project Momentum, wherein dozens of telekinetics were brought together ostensibly for the purpose of benefiting mankind. When corrupt FBI agents Dean Crept and Mike Mattress die, they are sent to hell, though they believe it must be an error. The story is based on the myth of King Arthur. A young, inexperienced squire Valiant, masquerading as Sir Gawain, is sent to accompany the Welsh princess Lady Ilene, a guest at Camelot, on her way back home.
