The Expendables, a group of elite mercenaries based in New Orleans, deploy to the Gulf of Aden to save hostages on a vessel from Somali pirates. The Expendables—led by Barney Ross and formed by Lee Christmas, Gunnar Jensen, and Toll Road—extract former member Doctor Death, a knives specialist and team medic, from a military prison during his transfer on a train. They recruit Doc to assist them in intercepting a shipment of bombs meant to be delivered to a warlord in Somalia. Arriving there, they reunite with Hale Caesar, who directs them to the drop point, where Ross is surprised to find out that the arms trader providing the bombs is Conrad Stonebanks, a former co-founder of the Expendables who went rogue and was presumed dead. In the ensuing firefight, The Expendables manage to kill all but Stonebanks, who shoots Caesar twice. As the team attempts to aid him, they are forced to retreat due to Stonebanks' advanced weaponry, and Caesar is severely injured in the process.

In Los Angeles, on January 15, 1947, LAPD officers Dwight 'Bucky' Bleichert (Josh Hartnett) and Lee Blanchard (Aaron Eckhart), investigate the murder and dismemberment of Elizabeth Short (Mia Kirshner), soon dubbed 'The Black Dahlia' by the press. Bucky learns that Elizabeth was an aspiring actress who appeared in a pornographic film. Through his investigation, Bucky learns that Elizabeth liked to hang out with lesbians. He goes to a lesbian nightclub and meets Madeleine Linscott (Hilary Swank), who looks very much like Elizabeth. Madeleine, who comes from a prominent family, tells Bucky that she was 'very close' with Elizabeth but asks him to keep her name out of the papers. In exchange for his silence, she promises him sexual favors. Continuing his relationship with Madeleine, Bucky meets her wealthy parents, Emmett (John Kavanagh) and Ramona (Fiona Shaw).

Picking up after the events of the original film, the people of the small town of Newt, Texas, led by Mayor Burt Hartman (Paul Rae), burn down the farmhouse of the Sawyer family for their role in aiding Jedidiah "Jed" Sawyer (Dan Yeager), also known as "Leatherface", in several murders. The arsonists are celebrated as heroes in the community, and the entire family is presumed dead. However, an infant, Edith Sawyer, is found by one of the townsmen, Gavin Miller (David Born), who promptly murders her mother, Loretta Sawyer (Dodie Brown), and adopts Edith with his wife Arlene (Sue Rock). Twenty years later, Edith has been raised by Gavin and Arlene as Heather Miller (Alexandra Daddario). One day, Heather is notified that her estranged grandmother, Verna Carson (Marilyn Burns), has passed away and left everything to her. Discovering that she was adopted, Heather, her boyfriend Ryan (Trey Songz), and two other friends, Nikki (Tania Raymonde) and Kenny (Keram Malicki-Sánchez), travel to Newt to collect her inheritance. Along the way, the group picks up a hitchhiker named Darryl (Shaun Sipos).
