The film is directed by Tom Kuhn and Fred Weintraub and written by Davis Miller, author of the books My Dinner with Ali and The Tao of Bruce Lee.

In Rome, Chen Ching-hua and her uncle Wang experience trouble with their restaurant from a mob boss who wants their property. When Chen refuses to sell, the mob boss sends gangsters to scare away customers. Appealing to an uncle in Hong Kong, Chen receives help in the form of Tang Lung, a martial artist. Disappointed, Chen asks how he can help her, but Tang confidently assures her that he is capable. At the restaurant, Tang learns that the staff have begun to learn karate, much to the annoyance of Quen, an employee who favors Chinese martial arts. Tang tells Quen that he should be open-minded and incorporate any moves that work. Before Tang can demonstrate his style to them, customers arrive, and the staff change clothes.

The Expendables—led by Barney Ross and formed by Lee Christmas, Gunnar Jensen, and Toll Road—extract former member Doctor Death, a knives specialist and team medic, from a military prison during his transfer on a train. They recruit Doc to assist them in intercepting a shipment of bombs meant to be delivered to a warlord in Somalia. Arriving there, they reunite with Hale Caesar, who directs them to the drop point, where Ross is surprised to find out that the arms trader providing the bombs is Conrad Stonebanks, a former co-founder of the Expendables who went rogue and was presumed dead. In the ensuing firefight, The Expendables manage to kill all but Stonebanks, who shoots Caesar twice. As the team attempts to aid him, they are forced to retreat due to Stonebanks' advanced weaponry, and Caesar is severely injured in the process.

The 1978 version uses portions of the original footage married to an entirely new plot involving a new character, Billy Lo,(盧比利) struggling against a racketeering "syndicate" after gaining international success as a martial arts movie star. San Francisco police officers John (Robert Jones) and Wong (Wong Tao) arrest two men raping a girl named Sylvia (Sylvia Chang), utilizing their knowledge of martial arts to take down the assailants, but at the station Sylvia refuses to press charges claiming that she knew the men and that the altercation was all in fun, in clear contradiction of what had truly taken place. In retribution for their arrest, the rapists kidnap John in broad daylight and take him to the beach, where he is beaten by numerous assailants. Wong manages to come to his partner's rescue, killing one of them. For this, he is kicked out of the force by his superior Captain Newman (Dan Ivan) and soon imprisoned, but John still keeps in touch with his old friend. After serving his sentence, Wong takes a job as a waiter. While waiting tables he meets Chuck Slaughter (Chuck Norris), a criminal mastermind. Slaughter offers Wong a job in his organization but Wong refuses, even as Slaughter threatens to have him killed.
