In 1974, at the Glensville Sanatorium, West Virginia, Dr. Brendan Ryan (Arne MacPherson) shows local psychiatrist named Dr. Ann Marie McQuaid (Kristen Harris) around the psychiatric hospital. He shows his three deformed patients, Three Finger, Saw Tooth and One Eye (are called themselves as The Hillickers), who all have the ability to sustain injury and not feel it. While Dr. Ryan and Dr. McQuaid move on, one of the mental patients grabs Dr. McQuaid's hair and rips out her hair clip, which she does not notice. The patient gives the hair clip to Saw Tooth. The Hillickers use the clip to pick the lock and escape from their cells. They release the other patients and together they kill the orderlies and the doctors, including McQuaid and Ryan.
Ariel Wolfe (Amanda Righetti) is the sister of Sara Wolfe (Ali Larter), a survivor of a publicity/birthday event hosted by Steven Price eight years ago in the Vannacutt Psychiatric Institute for the Criminally Insane. In the 1920s, the asylum was overseen by the sadistic psychiatrist Dr. Richard B. Vannacutt (Jeffrey Combs). Sara claimed that spirits of the house killed Price and the party guests, and that she barely escaped with her life. But no one believed her. She later commits suicide and a bereaved Ariel tries to find out why.
The film starts with a young man, named Travis, going into a hotel room where a Ukrainian couple, Victor and Anka, are currently staying. Anka and Victor fall unconscious after being drugged by the beer Travis gave them. It is then revealed that Travis is a member of the Elite Hunting Club. Victor later wakes up in a cell in an abandoned building, and watches as two guards drag Anka out of her cell.
A janitor, Jake Rogers (Cedric the Entertainer), wakes up next to a dead body with no memory of who he is and how he got there, but gradually comes to believe he is a secret undercover agent, and after fleeing the hotel and meeting his wife Diane (Nicollette Sheridan) and girlfriend (Lucy Liu) Jake goes on one crazy adventure to find out who he is.
Marion Crane steals $400,000 from her employer to get her boyfriend, Sam Loomis, out of debt. She flees Phoenix, Arizona, by car. While en route to Sam's California home, she parks along the road to sleep. A highway patrol officer awakens her and, suspicious of her agitated state, begins to follow her. When she trades her car for another one at a dealership, he notes the new vehicle's details. Marion returns to the road but, rather than drive in a heavy storm, decides to spend the night at the Bates Motel.
The film follows a man, who has an intellectual or mental disorder, living on a farm in rural Belgium. He demonstrates bizarre behavior from the beginning: fastening doll's heads to pigeons; collecting his feces in glass jars and beheading a hen for his own amusement. He is also obsessed with a sow who lives on the farm. We see him gleefully rolling around in the manure with the sow, and then he rapes it, which his behavior suggests he sees as an intimate and mutually agreeable act. Later, the sow gives birth to a litter of piglets. The man attempts to spoon-feed milk to the piglets, but the piglets prefer to drink directly from the milk bowl. In general, the piglets prefer their mother's company, repeatedly scorning the man's advances. Taking this rejection as an unforgivable personal slight, the man hangs the piglets to death and leaves their bodies strung up in the open. When the sow discovers the remains of the piglets, it runs madly around the farm squealing. The sow slips into a deep patch in the mud and drowns there.

This incident is covered up, and after being told her child was a stillbirth, Mary goes on a rampage through the hospital. The police find Mary wandering the grounds, and open fire on her after she throws the severed head of her rapist at them, ending her killing spree. "Mary Hatchet" subsequently becomes a folklore figure, with local youngsters celebrating the Mischief Night-esque "Blood Night" in her name.
Set in London, the film opens with American best-selling author Catherine Tramell in a speeding car with her companion, Kevin Franks, a famous English football star. Tramell takes the man's hand and begins masturbating with it, all the while increasing her vehicle's speed.
