Concert pianist Paul Orlac (Conrad Veidt) loses his hands in a horrible railway accident. His wife Yvonne (Alexandra Sorina) pleads with a surgeon to try and save Orlac’s hands. The surgeon transplants the hands of a recently executed murderer named Vasseur. When Orlac learns this, horror obsesses him. He is tortured by the presence of a knife he finds at his house, just like that used by Vasseur, and the desire to kill. He believes that along with the hands he has acquired the murderer's predisposition to violence. He confronts the surgeon, telling him to remove the hands, but the surgeon tries to convince him that a person’s acts are not governed by hands, but by the head and heart., 1h41

The film is biographical, telling the story of the life and artistic struggles of the French composer Hector Berlioz. Berlioz is shown as a recalcitrant medical student in an anatomy class dreaming of becoming a composer; at a demonstration during a performance at the Paris Opéra conducted by Habeneck; at supper with other young artists (Hugo, Janin, Dumas, Mérimée, Delacroix); and chasing after his future wife Harriet Smithson, after a performance of Hamlet. Also depicted are his life in a garret, while suffering from an illness due to an abscess in the throat; a visit from his mother who curses him; and the composition of the Symphonie fantastique. The film then shows his marital breakdown, the premiere of his opera Benvenuto Cellini, his travels throughout Europe, his second marriage to Marie Recio (called "Marie Martin" in the film), public acceptance in old age and reconciliation with his son., 2h2

The film frames the history of the "Red Violin" around a Canadian auction in 1997, where the violin is at the centre of multiple bids by interested parties, and a tarot card reading in 1681, where a violinmaker's wife has her future read for herself and her unborn child (and, by extension, that of the violin).
