Laure is a 10-year-old child whose family moves to a new address in Paris. One day Laure sees a group of boys playing outside the window and goes to play with them, but they disappear quickly. Instead, Laure meets Lisa, a neighborhood girl, and Laure introduces himself as Mikäel. Lisa then introduces Mikäel to the rest of the neighborhood children stating that "he" is the new kid in the apartment complex. A bath scene in the film reveals that Mikäel has a vagina, and a following scene confirms that Mikäel was assigned female at birth as his mom addresses him and his sister as "girls." Mikäel becomes friends with Lisa and the boys and tries to hide his genitalia to appear like a boy (make it seem that he has a penis). At one point this leads Mikäel to pee his pants, which originally makes him embarrassed, but is soon forgotten. As they all play, Lisa and Mikäel develop crushes on each other and after swimming one day, Lisa kisses him. At this point, Mikäel also seems to be accepted into the group of boys.

She initially considers an abortion. Going to a local clinic run by a women's group, she encounters outside a schoolmate who is holding a rather pathetic one-person pro-life vigil. Once inside, however, Juno is alienated by the clinic staff's authoritarian and bureaucratic attitudes. She is particularly offended by their calling her "sexually active", a term which she feels demeans the highly emotional event by which she became pregnant. All of this decides her against abortion, and she decides to give the baby up for adoption. With the help of her friend Leah (Olivia Thirlby), Juno searches the ads in the Pennysaver and finds a couple she feels will provide a suitable home. She tells her parents, Mac (J.K. Conceived by means of in vitro fertilization, Anna Fitzgerald (Abigail Breslin) was brought into the world as a savior sister at the informal suggestion of Kate's doctor, Dr. Chance (David Thornton) (a formal suggestion from the doctor would have been a violation of legal and medical ethics). As a genetic match for her older sister, Kate (Sofia Vassilieva), who suffers from acute promyelocytic leukemia, to donate compatible organs, blood and tissue in order to keep her sister alive. Her family members are introduced one by one, and each tells about how Kate's illness has affected them personally. When Kate turns 15, she goes into renal failure. Eleven-year-old Anna knows that she will be forced by her parents to donate one of her kidneys. She also realizes that she may not be able to live the life she will want to lead - she may be unable to cheerlead, play soccer, or be a mother. Anna tells her parents that she does not want any of this and proceeds to sue them for medical emancipation and the rights to her own body. Her domineering mother, Sara (Cameron Diaz), who leads an obsessive campaign to keep Kate alive, is indignant at Anna's decision when she receives the notice of court proceedings. Attorney Campbell Alexander (Alec Baldwin) agrees to work for Anna as her guardian ad litem, suing for partial termination of parental rights. It is later learned he agreed to take the case not for the notoriety, but because he suffers from epilepsy, and is genuinely sympathetic to her predicament.
