Synopsis
In a flashback to 1991, while riding his bike through the woods, a young boy spots a woman darting between the trees. He puts down his bike and runs after her. The woman meets up with another woman, they speak briefly and run together through the woods until they arrive at an old stone house. The boy continues after them, peeks through the window of the house and watches as they wildly slaughter a rooster. The women spot the boy at the window and chase after him, saying "You'll be sorry, I'll kill you, You'll never get away from us". The flashback ends.Actors

Detective Anna Manni (Argento) travels to Florence on the trail of a serial killer Alfredo (Thomas Kretschmann). While at a museum, Anna is struck by Stendhal syndrome, which causes people to become overwhelmed by great works of art. Alfredo uses this disorder against Anna, kidnapping and raping her. She escapes and is deeply traumatized. Alfredo tracks her movements and is able to capture her again. This time, Anna manages to break free, badly wounding her captor, and knocking him into a river.

Suzy Bannion, an American ballet student, arrives from her flight in Munich, Germany on a stormy night to enroll in a prestigious dance academy in Freiburg. After Suzy is unable to gain access into the academy, she spends the night in town. Pat Hingle, a student who is expelled from the academy is seen by Suzy leaving and looking frightened. Pat finds refuge at a friend's apartment in town. After she locks herself in the bathroom, a mysterious arm smothers her against the glass. Her friend hears this and tries to scream for help but Pat is repeatedly stabbed and bound with a cord before she is hung in mid-air. Her friend is killed directly below by the falling glass and metal.

Aura (Asia Argento), a young woman suffering from anorexia escapes from a psychiatric hospital and meets a young man, David (Christopher Rydell), who offers to let her stay with him rather than go back to the hospital. However, Aura is soon caught, but her return to the hospital coincides with the start of a string of murders of hospital staff members, past and present. The film centers around a serial killer known as "The Card Player", who is kidnapping young women in Rome. Franco Arnò (Karl Malden) a middle-aged blind man is walking down a street at night with his niece Lori (Cinzia De Carolis) when they pass by a man in a car. Franco overhears him mention blackmail. They walk back to Franco's apartment nearby and while Lori sleeps, Franco works on a puzzle when he hears a noise. Outside, the unseen man in the parked car gets out and knocks out a night watchman by the gate of a large medical complex, the Terzi Institute. The unseen man uses a screwdriver to get inside the building. A doctor preparing to leave for the night sees the night watchman unconscious and sees a dark figure running away.

Roberto Tobias (Michael Brandon) plays the drums for a local rock and roll band when at various time, he sees a man in dark sunglasses wearing a suite and tie, watching him. After the session, Roberto follows the man through the dark streets to an apparently abandoned opera house where he confronts the man and asks him why he's been following him for the past several weeks. The man declines such actions and pulls a knife on Roberto when the drummer gets too close. In the struggle, the man is accidentally stabbed and he falls from the stage to the lower level. Suddenly a spotlight is turned onto Roberto and a masked person on the balcony snaps some photos of Roberto holding the bloody knife.

Rose Elliot (Miracle), a poet living alone in New York City, finds an ancient book called The Three Mothers. It tells of three evil sisters who rule the world with sorrow, tears, and darkness. The book, written by an architect named Varelli, reveals that the three dwell inside separate homes that had been built for them by the architect in Rome, Freiburg, and New York. Rose suspects that she is living in one of the buildings and writes to her brother Mark (McCloskey) in Rome, urging him to visit her. Using clues provided in the book as a guide, Rose searches the cellar of her building and discovers a hole in the floor which leads to a water-filled ballroom. She accidentally drops her keys and enters the water to find them. After she reclaims the keys, a putrid corpse suddenly rises from the depths, frightening her.
