Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.

Berlin techno DJ and producer Martin Karow (nicknamed Ickarus) is touring the techno clubs of the world with his girlfriend Mathilde while working on a new studio album that he plans to release soon. In order to be able to work and party day and night, Ickarus takes all kinds of drugs, mainly supplied by his friend Erbse at the clubs in Berlin. After consuming a PMA-containing ecstasy tablet, Ickarus goes into a drug-induced psychosis, eventually finding himself naked in a Berlin hotel where his antics attract the attention of the hotel staff. Governor Jaisingh Sinha (Raj Babbar) is a popular and prominent political figure of the state, besides being well known as an upright person. One day, Gov. Sinha presides over a meeting on the issue of "Open Door Policy", a scheme which will pave the way for foreign companies to operate in India, by implementing usage of computer, industrial machinery etc. The meeting is attended by influential industrialist Meghnad Chaudhry (Dalip Tahil), Union leader Vilas Rao (Sharat Saxena) and minister Mantriji (Prem Chopra), among others. While Chaudhry is in favour of the policy because it would generate foreign investment in India, Vilas Rao strongly opposes the policy which would bring with it the unemployment of a large number of workers because of the utilization of computers and industrial machinery. The difference of opinion of Chaudhry and Vilas Rao leads to a heated argument between the two for which Gov. Sinha terminates the meeting without arriving at any decision on the policy.
