Lola Friedlander est une avocate connue. Sa vie, se partage entre son activité professionnelle, sa famille juive envahissante, son fils Bolivar et Ferdinand et son amant journaliste. A man who had a stroke 17 years ago (Hume Cronyn) is left incapacitated and bed-ridden. He has been cared for by his daughter Bessie (Diane Keaton) in their Florida home, and totally ignored by his other daughter, Lee (Meryl Streep), who moved to Ohio with her husband 20 years ago and has never contacted her family. Now, however, Bessie's doctor has informed her that she has leukemia and needs a bone marrow transplant and she turns to her sister for help. Lee, in turn, turns to her son Hank (Leonardo DiCaprio), who has been committed to a mental institution for setting fire to his mother's house. When Lee finds that she may have to take over her father's care, she at first begins shopping around for nursing homes. Eventually, however, the once-estranged family grows close.

The film begins in the spring and documents Eads' life through the following winter. Eads falls in love with Lola, a transgender woman. He spends those remaining warm days in the company of his "chosen family": Maxwell, Cas, and "the rest". That summer, his mother and father drive ten hours to visit Robert, who is still their daughter in their eyes. Later that year, Eads makes his last appearance at the Southern Comfort Conference in Atlanta, Georgia, a prominent transgender gathering. Already feeling ill, he addresses a crowd of 500 and takes Lola to what is for them a prom that never was. Shortly after the conference, Eads dies in a nursing home with his chosen family.
