The real Chikuzan appears on a stage in a small theatre, Shibuya Jean-Jean, and begins telling the story of his life. The scene changes to his childhood. Sadazo (Chikuzan's real name) becomes partially blind due to illness at the age of three. Growing up he is bullied. Nami Matsushima is found in a wedding chapel by police led by detective Hirose. They handcuff her, but she is able to escape. Kudo, a worker in a sex show club, rescues her. He is a radical with a history of problems with the police. One of the women from the sex show, who had unsuccessfully tried to seduce Kudo, finds Nami's handcuffs in Kudo's things, and informs the police. Nami Matsushima (Meiko Kaji) is set up by her boyfriend, a crooked police detective named Sugimi (Isao Natsuyagi) to win favor with the Yakuza. She is raped by several drug dealers. She makes a failed attempt to stab Sugimi on the steps of the Tokyo Metropolitan Police Headquarters. She is sentenced to do hard time in a women's prison. Matsushima is given the number 701. The prison is run by sadistic and lecherous male guards. The prisoners are forced to walk up and down a stair-like contraption naked with male guards watching from below. While in prison she meets inmates like Yuki Kida (Yayoi Watanabe) who was committed for fraud and theft, Otsuka (Akemi Negishi), jailed for burglary and extortion, and Katagiri (Rie Yokoyama) who has been impounded for arson and illegally disposing of a body. On the outside, Sugimi and the Japanese mafia orchestrate a plan in which Matsushima will succumb to an "accidental" death in prison. They enlist the help of Katagiri, pulling on her ties to both Sugimi and the mafia, and quickly set their plan in motion.

The first to be introduced is a mentally challenged boy who lives in a world of fantasy in which he is a tram conductor. He is both the tram and the tram driver and follows a set route and schedule through the dump; his dedication to the fantasy is fanatical. The film title refers to a Japanese onomatopoeia for the sound made by a tram or train while in motion ( "Do-desu-ka-den do-desu-ka-den do-desu-ka-den"). The sound is made by the boy as he makes his daily faux-tram route through the dump.

The film takes place in Koishikawa, a district of Edo (the former name of the city of Tokyo), in the 19th century. Young Dr. Noboru Yasumoto (Yūzō Kayama) is the film's protagonist. Trained in a Dutch medical school in Nagasaki, the arrogant Yasumoto aspires to the status of personal physician of the Shogunate, a position currently held by a close relative; his father is already a well-established, highly competent physician. Mr. Tako, head of Pacific Pharmaceuticals, is frustrated with the television shows his company is sponsoring and wants something to boost his ratings. When a doctor tells Tako about a giant monster he discovered on the small Faro Island, Tako believes that it would be a brilliant idea to use the monster to gain publicity. Tako immediately sends two men, Sakurai and Kinsaburo, to find and bring back the monster. Meanwhile, the American submarine Seahawk gets caught in an iceberg. The iceberg collapses, unleashing Godzilla (who, in the Japanese version, had been trapped within since 1955), who then destroys the submarine and a nearby military base.
