Nu Image is an American film company started by Avi Lerner, Trevor Short, Danny Dimbort and Danny Lerner in 1992. The company has made to date mostly action films. Many of their films are often filmed in South Africa and Bulgaria, among other parts of the world. In 1996, the company launched a subsidiary label called Millennium Films.

The film tells the story of two people with Asperger syndrome (a form of autism). In the opening, a collision between two alien spaceships occurs. The collision unleashes a capsule which heads toward Earth. The capsule collides with a small ship, causing an explosion. The capsule lands near an undersea research lab, the Oshona. The capsule, full of red crystals, attracts sharks. The sharks eat the crystals which cause them to become vicious killers, eating anyone and attacking anything that comes near them. Now, the researchers of the Oshona are stuck underneath the Bermuda Triangle. The Navy attempts to rescue them, but many people die in the process.

Kyle LeBlanc (Van Damme) is an American working overseas in Russia. When he hears his wife being attacked over the phone, Kyle rushes home to find that he is too late. The man who has killed his wife is found not guilty on lack of evidence (but in fact because the defense bought the judge). Kyle, therefore, takes the law into his own hands and kills the man to avenge his wife's death, and for this he is sentenced to life in prison without parole. He soon befriends inmate Billy Cooper, a 21-year-old who is subjected to constant rape and beatings by prison inmates (they are actually helped by the guards who lock him in cells with these prisoners during the night) and also Malakai, a wheelchair-bound prisoner who claims to know the ins and outs of prison life.

The film begins with a group of divers installing a power cable at the bottom of the Pacific Ocean, when a shark swims up and kills one of them. Six months later, lifeguard Ben Carpenter (from the first Shark Attack film) and his partner Esai drive out to sea to catch some lobster. While diving, Ben finds a broken power cable with a large shark tooth stuck in it. After he pulls out the tooth, he is caught by two other divers, and soon goes back to land. Later he posts a description of the tooth online, but as he cannot find a matching shark tooth on the internet, he names it "mystery shark". Natural history researcher named Cataline "Cat" Stone gets the message, finds him and looks at the tooth. She believes it to come from a huge prehistoric shark called Megalodon (Greek for "big tooth").

One week later, Dr. Nick Harris and his assistant get the shark and install it as a new attraction at Water World - a Sea World rip-off. When the shark escapes, Nick and Samantha go to hunt it and discover that there are 6 mutant Great Whites living in a cave near the beach, and the Water World's surfing competition might be the next victim! Now they team up with Roy - a Discovery Channel shark hunter - to kill the beasts and save Cape Town. Eventually they succeed in luring the sharks into their cave and blow it up, killing them. Roy is initially believed to have perished but in reality survived.

Eight teenagers, including Brady (Mark McLachlan), Claire (Caitlin Martin), Duncan (Chris Solari), Kit (D. W. As the group are about to depart on their boat, they are warned by Sheriff Bowman (Harrison Young) to be sensible and keep out of danger. After a day of partying, the group have a bonfire, where Kit tells them a local story about how in the early 1900s, 96 years ago, a hotel owner named Harlan featured a crocodile named Flat Dog at his hotel. He goes on to say that Harlan eventually sets up a shrine to Flat Dog, believing her to be an avatar to the ancient Egyptian crocodile god (Sobek), creating a cult that worshiped the crocodile. The town eventually ran Harlan away because of his heathenism and torched his hotel years later when Kit was a kid. Close by, two local fishermen destroy a crocodile nest, only for them both to be attacked by Flat Dog, who devours them both.
