Pays was born in London, England, the daughter of Jan (née Miller), an actress, and Howard Pays, a talent agent and former actor. She is the niece of former child actress Mandy Miller, after whom she is named. Her first marriage, from 1984 to 1987, was to Peter Kohn, who later went on to be the production manager for Pirates of the Caribbean. Following her divorce, Pays married American actor Corbin Bernsen on 19 November 1988, with whom she has four sons. Peter Mandrake (Coyote), an American photographer in Brazil, is preparing an in loco essay for his new book, about the "Train Surfers" (groups of boys who court danger "surfing" on the roof of the trains) in the city of Rio de Janeiro. A local call girl he is friends with is murdered, and when the police can make no progress Mandrake decides to investigate himself. Subsequently, two hired thugs break into his apartment demanding a disk, and, when he doesn't produce it because he can't do so, they rape his girlfriend and stab him, leaving him to die. Vowing revenge, Mandrake enlists the help of Hermes (Karyo), a professional knife fighter who owes Mandrake a debt, to teach him the art of knife fighting. The obsession this develops into causes Mandrake's girlfriend to leave him, wanting the whole thing to simply go away, but Mandrake refuses to let go.

Although set during the war, the tale is more of a mystery than a story about the war. Sergeant First Class Buck McGriff (Willem Dafoe) and Sergeant First Class Albaby Perkins (Gregory Hines) are two joint services Criminal Investigation Division (CID) agents on duty in war torn Saigon. When a prostitute is found murdered they discover that the prime suspects are high ranking U.S. Army officers. As they investigate they find that there have a string of at least six murders in the last year, but the previous inquiry was shut down from higher up the chain of command. Investigations lead them to Colonel Dexter Armstrong (Scott Glenn), but Armstrong rules himself out of inquiries by committing suicide. With the help of a French nun Sister Nicole (Amanda Pays) and their non-commissioned officer in charge, Master Sergeant Dix (Fred Ward), they finally close in on their target. As their investigation leads them closer and closer to the murderer, they find their lives are in danger and they end up nearly being sent home. The movie ends with an unexpected twist when they rule out all their suspects by conducting an interview in a Viet Cong tunnel base, and their NCO is the killer.
