A diamond heist goes very wrong, leaving one of the team of criminals dead and another arrested for his murder. The surviving thief, Eddie Cordero, wakes up in a hospital being guarded by Constable Frances Jane. He tries to con her into letting him go, but she's too smart for him and won't fall for his various pleas. Later in the evening, Cordero's partners in the diamond heist break into the hospital and head for his room. In the ensuing chaos, Constable Jane ends up having to work with Cordero to survive the night while at the same time trying to keep him from escaping.

Film student Alex Wright and his friends Jennifer Parker, Trevor Thompson, Tessa Hamill, and Jared Lee all decide to produce a documentary about the original Grave Encounters movie, which the entire public aside from Alex believes to be fake. Alex posts an online plea for any information about the film and receives a message from someone named "DeathAwaits6". The message leads him to the mother of Sean Rogerson, the actor who played Lance in Grave Encounters. She believes that Sean is still alive but they discover that she has severe dementia.

Jerry Hartfield, the producer of Grave Encounters, a famous ghost investigation television series, explains that Grave Encounters was cancelled in 2003 after the fifth episode, when the cast and crew, along with the footage for episode six, mysteriously went missing. Paul and Jessie Duncan (Greg Kinnear and Rebecca Romijn) are a happily married couple who have an eight-year-old son named Adam (Cameron Bright). The day after his eighth birthday, when fetching a basketball he was given into the street, Adam is killed in a collision. While leaving a church, Jessie and Paul are confronted by Dr. Richard Wells (Robert De Niro), an old professor of Jessie's. He offers to clone Adam, an illegal procedure which would require a change of location and identity, to which the Duncans reluctantly agree. Everything appears to be fine with the new Adam until he reaches his eighth birthday. That night, he experiences a violent nightmare. Richard explains to Paul that it is typical for boys his age to have night terrors, and that it is not serious. He explains that because Adam II has reached the age at which the original Adam died, his life cannot be predicted anymore. From that moment on, Adam II continues to have night terrors until they become visions and he starts having them when he's wide awake, losing control of his actions.

Barry is a private with the 101st Airborne Division of the United States Army, stationed at Fort Campbell, Kentucky. Calpernia works as a showgirl at a transgender revue in Nashville, Tennessee when the two met in 1999. Barry's roommate Justin Fisher (Shawn Hatosy) brings Barry to the club where she performs. When Barry and Calpernia begin seeing each other regularly, Fisher begins spreading rumors on base about their relationship, which appeared to be a violation of the military's "don't ask, don't tell" policy about discussing the sexual orientation of military personnel. Barry faces increasing harassment and pressure, which explode into violence over Fourth of July weekend. While Calpernia performs in a pageant in Nashville, Barry is beaten to death in his sleep with a baseball bat by Calvin Glover, who had been goaded by Fisher into committing the crime. The film ends with a discussion of the aftermath.

In Boston in the early 1960s, Geraldine Cummins was walking home alone from the movies when she was jumped and raped by a black man. Stunned, she returned home to her husband Bob, stating she had been raped. Sometime later, she finds she is pregnant. In the beginning she hates the thought of her unborn baby, not wanting a constant reminder of her rapist, but keeps it for a few reasons: she is Catholic and she harbors a small hope that it could be her husband's baby. Her husband is supportive during the pregnancy. As the months pass, Gerry comes to be attached to the baby, later stating "8 months is too long to close your heart to a piece of yourself." She becomes fearful of what will happen if her baby is black, the social isolation it would receive and what people would think of her. After going into labor and confiding in a doctor her situation, she becomes convinced that it would be best if she gave it up. The baby is a black girl. She names her newborn daughter Barbara Anne Cummins and gives her to foster mother Corrine Burrel, a black woman in Roxbury, a black neighborhood. Gerry is heartbroken to give up her daughter.
