Hekiro et Gen sont frère et sœur, ils s'entendent bien malgré un foyer peu chaleureux. Le père, un écrivain qui connait un certain succès, passe ses journées enfermé dans son bureau à travailler ses manuscrits. La belle-mère, une bigote chrétienne, est atteinte d'une forme sévère de rhumatismes qui la fait souffrir du pied et de la main gauche. Gen est une jeune fille au fort tempérament et responsable, elle supplée sa belle-mère handicapée dans les tâches quotidiennes et s'occupe avec affection de son frère Hekiro. Ce dernier, ne trouve pas sa place dans ce foyer et fait les 400 coups, entre bagarres au lycée, vol, dettes qu'il contracte dans une salle de billard ou chez un loueur de hors-bord. Cependant, malgré la confiance entre frère et sœur, certains problèmes surgissent. Quand Hekiro est atteint d'une tuberculose incurable, il s'attriste de voir sa sœur envisager de se marier. Le jour de sa mort, elle tâche de supporter courageusement le chagrin et la fatigue.

Zatoïchi porte secours à une voyageuse en train d'accoucher et qu'un yakuza vient de détrousser. Il sauve le bébé, mais la mère meurt en prononçant le nom du père : Sataro, à Shiobara. Zatoïchi prend le nourrisson sous sa protection, mais un mystérieux enfant l'épie jusqu'à Shiobara où il ne trouve que la sœur de Sataro, Oyaé, harcelée par des yakuzas. Elle attend son frère, d'une province voisine, qui doit apporter l'argent destiné à payer sa dette. Quand Sataro se fait enfin expliquer les circonstances du drame, l'enfant (qui n'est autre que son fils aîné) réapparaît et accuse le masseur aveugle du meurtre de sa mère...

One year after the first film, Zatōichi travels back to the town near the Joshoji Temple, to pay respects at the grave of Hirate, the samurai he killed. Three brigands attack Zatōichi while he dries his clothes, and are despatched by a one-armed swordsman. Later that day, Zatōichi is hired to massage a powerful lord who, unbeknownst to all but the lord's highest retainers, is insane. Zatōichi observes the nobleman's unstable mental condition, and the retainers decide to kill him. Zatōichi defeats the first three attackers, and retires to a restaurant. The attack having failed, the lord's men hire local yakuza (gangsters) to finish the job. Learning of this, Zatoichi remarks to himself that he would have kept quiet if they had just asked him to do so.

The film features Raizo Ichikawa as Tange Tanzen, and Shintaro Katsu as Yasubei Nakayama (later becoming Yasubei Horibe when he is adopted into another family), and opens with Yasubei simultaneously dueling several members of a rival dojo (school of swordsmanship). Yasubei deftly beats his opponents, but is expelled from his school as a result so as not to create enmity between the two schools. Tange, who is the best swordsmen of the defeated school, is expelled from the school because he was present at the duel but failed to intervene to help his fellow dojo-mates. He argued that he could not interfere because he was then on shogunate duties, but his colleagues accused him of cowardice and he was thereby expelled. After his expulsion, Tange comes across the maiden Chiharu, who was being attacked by feral dogs. Tange protects Chiharu (a member of the Uesagi Clan) and in the process kills one of the attacking "noble" dogs. However, the act carries with it the penalty of death due to the current Shogun's affinity for dogs. Yasubei helps to cover up Tange's act, but only as a way to become closer to Chiharu. It is because of his hidden affection for Chiharu that Yasubei considers aligning with the Uesagi clan (which is loyal to Chamberlain Kira who is later targeted for a revenge killing by the 47 Ronin).
