From the moment he was six, Max wanted to die. He tried shooting himself, hanging himself and even throwing himself out of a window. But somehow, he always managed to survive. She tries overdosing on pills, running in front of a truck and jumping from a bridge, but luckily each time, Max comes to her rescue. Finally, he brings Grace to her mother and in a bizarre turn of events, the terminally ill oddball orders them to dig her grave. Mom says her goodbyes, but not before she tells Max and Grace that they both have something to live for—she sees a vision of their baby girl in Grace's tummy. It's more than the hope Grace and Max have been searching for; it's the wonderful beginning of a whole new life.

A suicidal Hunter "Patch" Adams (Robin Williams) commits himself into a mental institution. Once there, he finds that using humor to help his fellow patients gives him a purpose in life. Because of this he wants to become a medical doctor and two years later enrolls at the Medical College of Virginia (now known as VCU School of Medicine) as the oldest first year student. He questions the school's soulless approach to medical care and clashes with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients as patients and not bond with them as people. Because of this and incidents such as setting up a giant pair of legs during an obstetric conference, he is expelled from the medical school, although he is later reinstated due to his methods actually helping patients improve. Adams encourages medical students to work closely with nurses, learn interviewing skills early, and argues that death should be treated with dignity and sometimes even humor.

In New York City, Nick Cassidy (Sam Worthington) checks in to the Roosevelt Hotel under the false name of Walker, goes to his hotel room on the 21st floor, and climbs on the ledge, apparently ready to commit suicide. The crowd below calls the police, with Dante Marcus (Titus Welliver) controlling the crowd, while Jack Dougherty (Edward Burns) tries to talk with Nick. However, Nick will only speak to negotiator Lydia Mercer (Elizabeth Banks), who is on a leave of absence, after failing to save a suicidal policeman. Lydia arrives at the hotel room and manages to acquire Nick's fingerprints from a cigarette they share. Dougherty has them analyzed and discovers that Nick is an ex-policeman, arrested for stealing the $40 million Monarch diamond from businessman David Englander (Ed Harris). Nick was given a 25-year sentence but escaped from the Sing Sing Correctional Facility one month earlier, after being allowed to attend his father's funeral. Nick, however, maintains his innocence and accuses Englander of framing him for the theft of the diamond, as Englander lost his fortune and was too proud to sell the diamond.
