In 1969, Dr. Malcolm Sayer (Robin Williams) is a dedicated and caring physician at a local hospital in the New York City borough of The Bronx. After working extensively with the catatonic patients who survived the 1917–1928 epidemic of encephalitis lethargica, Sayer discovers certain stimuli will reach beyond the patients' respective catatonic states; actions such as catching a ball, hearing familiar music, and experiencing human touch all have unique effects on particular patients and offer a glimpse into their worlds. Leonard Lowe (Robert De Niro) proves elusive in this regard, but Sayer soon discovers that Leonard is able to communicate with him by using a Ouija board.

Babu is a care-taker in a mental hospital who shows much devotion to his job unlike other employees and more affection towards the patients in the hospital. He wears the patients' dress and moves with them like one among them. Due to this nature the Chief Doctor, also a priest Father, likes him very much than others fetching Babu the enmity of two compounders in the hospital. One of the financial donors of the hospital brings a wandering girl to hospital suspecting that she is mentally ill. Since nobody knows about her anything and she is a mentally challenged the hospital admits her. Babu moves with the girl like he moves with all other patients but shows more affection than others as she does not have anybody to take care. The employees watch with curiosity and spin stories on them. Babu names the girl as Meenakshi and calls her Meenu. One of the two compounders tries to rape a mentally challenged girl and Meenu makes snake alert to stop it which fetches everybody. This angers the compounder and he gives electric shock. Babu rescues Meenu and complains to Father resulting in the suspension of the compounder and his friend. Babu now makes more care for her and spends more time with her.

Elaine walks in, and Freddy makes the attack appear as a suicide attempt and has her admitted to the hospital for help. Going into hysterics after an attempt to sedate her, Kristen is calmed when Nancy Thompson, a newly hired member of the hospital staff arrives and earns her trust by finishing Freddy's nursery rhyme.

The staff and patients go about their daily business at Buenos Aires' José Borda Psychiatric Hospital on a summer day in 1985. A staff psychiatrist, Dr. Julio Denis (Lorenzo Quinteros) is surprised to hear that his ward for non-violent delusional cases has one patient too many. Denis finds him in the chapel playing the organ like a virtuoso. Summoning him (Hugo Soto) to his office, Denis finds the man's speech is measured and articulate as he explains his presence there as a result of an image being projected from light years away. He introduces himself as "Rantés" (an exotic sounding name in Argentina). Dr. Denis is convinced that Rantés is a fugitive hoping to hide from the law in the hospital. He lets the patient stay however, after seeing how his caring touch helps the other patients. The doctor is amused by his extraterrestrial claims and he suspects that the man is a genius using his talents as a charade.
