Johnny Walker (Mickey Rourke) is a down-and-out boxer with brain damage who has recently moved into a sea-side resort. He falls in love with Ruby, a carnival owner who has a lot in common with Johnny. He also befriends Wesley Pendergrass (Christopher Walken), a corrupt promoter. Wesley and Johnny form a strong friendship, and it's clear that Johnny comes to idolize Wesley who wants to use Johnny as muscle in a robbery and asks for his help. Johnny has to choose between the love of Ruby (Debra Feuer) or the friendship of Wesley.

In 1985, Ivan Drago (Lundgren), a muscular 6-foot-5, 261-pound Soviet boxer, arrives in the United States with his wife Ludmilla (Nielsen), and a team of trainers from the USSR and Cuba. His manager, Nicolai Koloff (Pataki), takes every opportunity to promote Drago's athleticism as a hallmark of Soviet superiority. Motivated by patriotism and an innate desire to prove himself, Apollo Creed (Weathers) challenges Drago to an exhibition bout. Rocky (Stallone) has reservations, but agrees to train Apollo despite his misgivings about the fight. He asks Apollo whether the fight is against the Russian, or "you against you".

In the four and a half years since winning the world heavyweight title, Rocky Balboa has had a string of successful title defenses (10) and has seen his fame, wealth and celebrity increase. In a brief scene in 1964, an aging, overweight Italian American, Jake LaMotta (Robert De Niro), practices a comedy routine. The rest of the film then occurs in flashback. In 1941, LaMotta is in a major boxing match against Jimmy Reeves, where he received his first loss. Jake's brother Joey LaMotta (Joe Pesci) discusses a potential shot for the middleweight title with one of his Mafia connections, Salvy Batts (Frank Vincent). Some time thereafter, Jake spots a 15-year-old girl named Vickie (Cathy Moriarty) at an open-air swimming pool in his Bronx neighborhood. He eventually pursues a relationship with her, even though he is already married. In 1943, Jake defeats Sugar Ray Robinson, and has a rematch three weeks later. Despite the fact that Jake dominates Robinson during the bout, the judges surprisingly rule in favor of Robinson and Joey feels Robinson won only because he was enlisting into the US Army the following week. By 1947, Jake marries Vickie.

Following the fight at the end of Rocky, in which Apollo Creed has successfully defended his heavyweight title in a split decision, he and Rocky are taken to the same hospital. Apollo challenges Rocky to a rematch, but Rocky declines and announces his retirement. His girlfriend Adrian supports this choice, as do his doctors, who reveal he will require surgery for a detached retina, which could lead to permanent blindness. After Rocky is discharged from the hospital, he enjoys the benefits of his life's changes: Rocky's new fame attracts an agent who sees Rocky as a potential endorsement and sponsorship goldmine, and his sudden wealth encourages him to propose to Adrian. She happily accepts, and they marry in a small ceremony. Soon after, Rocky and Adrian happily learn that Adrian is pregnant.
