The film takes place in 1986, 13 years after the events of the first film. It begins with Buzz and Rick, two rowdy high school seniors racing along an abandoned stretch of Texas highway, en route to a weekend of fun in Dallas. They are heavily intoxicated and use their car phone to call and harass on-air radio DJ Vanita "Stretch" Brock (Caroline Williams). Unable to convince them to hang up, Stretch is forced to keep the line open. Buzz and Rick encounter a large pickup truck which runs parallel to them on a remote bridge. Suddenly, Leatherface (Bill Johnson), wielding a chainsaw, emerges from the back of the truck and proceeds to attack Buzz and Rick. After a short struggle, Rick tries to shoot Leatherface with a .44 Magnum revolver, but misses his target. Leatherface then slices off part of the driving Buzz's head, and the car ends up crashing and killing Rick.

A plague in the form of a toxic viral gas is unleashed at major sporting events across the United States. The gas turns its victims instantly "into an immediate rage of insanity and violence". Controversial Los Angeles talk show radio host Logan Burnhardt and his production team are caught up in the middle of the chaos. Only blocks away from the explosion site, they begin to receive reports of rioters in the streets and listeners continue to call in reports of their first hand experiences. An elite group of soldiers led by Mack (Cuba Gooding, Jr.), are sent by a CIA agent named Elissa Cardell (Valerie Cruz) to rescue her father, a scientist named Wesley (Ron Perlman), who is working on an archaeological dig site in the Middle Eastern desert. When they enter the dig site, which is a large underground laboratory, they encounter a priest who has strange boils all over his body. The team's medic, Doc (Taryn Manning) sedates the priest. Click (Brandon Fobbs) uses the dig site's terminal to activate the elevator and notices something codenamed the Gehenna Project and asks Cardell if she knows what it is but she denies knowing about it. Mack orders Nickels (Zack Ward) to stay and watch the priest while the rest of the team takes the elevator deeper into the dig site.

Picking up immediately where the first film ended, Marybeth (Danielle Harris) is attacked by Victor Crowley (Kane Hodder), but manages to escape. Jack Cracker finds her, pulls her out of the river, and takes her back to his cabin. After finding out her last name, he forces her to leave, telling her that if she wants help, she needs to see Reverend Zombie (Tony Todd). Moments after she leaves, Jack views Shapiro's camcorder, which he found in the swamp. The footage shows Shapiro filming different topless women, including Misty and Jenna. After Jack finishes watching the video, he is killed by Victor Crowley, who gouges his intestines out and strangles him with them until he decapitates him.

While drunk and out hunting one day, Ike accidentally shot his pregnant wife in the eye, and since then he has been coerced by hallucinations or ghosts of his wife and unborn son into atoning for the act by acquiring replacement eyes. Whenever a woman he finds suitable stops at his gas station on Exit 33, Ike has them use a sabotaged pump that causes their vehicle to break down a short distance away. Ike then drives up, kills anyone with the woman, knocks her out and takes her back to the gas station, where he gouges out their eyes, and makes jerky out of their body. Ike preserves the eyes, and places them in one of the mounted hunting trophies in his den.
