As a boy, a reclusive and antisocial Sufferton resident, Max Seed, was disfigured in a school bus crash that killed everyone else involved in it. In 1973, Seed began torturing and murdering people, filming some of his victims starving to death in his locked basement, and ultimately racking up a bodycount of 666. In 1979, Seed is arrested by Detective Matt Bishop in a siege that claims the lives of five of Bishop's fellow officers. Seed is sentenced to death by electric chair, and incarcerated on an island prison, where he is a model inmate, only acting out when he kills three guards who try to rape him.

In the fictional town of Tenderville, Oregon, Bill Williamson, a 23-year-old man, is living with his parents and working a low-paid job as a mechanic, feeling bombarded with the problems of the world, by ubiquitous TV sets, radios, and the outspoken political views of 21-year-old Evan Drince, who seems to be his sole friend. Bill's parents ask him to move out and Bill leaves for work, stopping to get coffee along the way, and argues with the shop owner when he is unsatisfied with his coffee. At work, his boss interrupts him while Bill is working on a personal vehicle off the clock. The boss then condescendingly dismisses Bill's request for a raise.

Ron Decker, a young man convicted for drug possession, is sent to prison where veteran con Earl Copen takes Decker under his wing and introduces him into his own gang. Danny Vinyard (Edward Furlong), a high school student and budding neo-Nazi in Venice Beach, California, receives an assignment from Mr. Murray (Elliott Gould), his history teacher, to write a paper on "any book which relates to the struggle for human rights". Knowing Murray is Jewish, Danny writes his paper on Adolf Hitler's Mein Kampf. Murray attempts to get Danny expelled for doing this, but Principal Dr. Bob Sweeney (Avery Brooks) — who is black — refuses, instead informing Danny that he will study history and current events under Sweeney, and that the class will be called "American History X". Danny's first assignment is to prepare a paper on his brother Derek (Edward Norton), a former neo-Nazi leader who has just been released from prison after serving three years for voluntary manslaughter. Danny is warned that failing to submit the paper the next morning will result in his expulsion. The rest of the film alternates between a series of vignettes from Danny and Derek's shared past (distinguished by being shown in black and white), and present day events (shown in color).

Philadelphia, 1999. In a violent home invasion, engineer Clyde Shelton (Gerard Butler) is forced to witness the rape and murder of his wife and young daughter by Clarence James Darby (Christian Stolte) and his accomplice Rupert Ames (Josh Stewart). Prosecutor Nick Rice (Jamie Foxx) is unable to use DNA evidence to securely convict both accused. Unwilling to take a chance on lowering his high and yet unbeaten 96% conviction rate, he chooses to make a deal with Darby (the actual murderer), letting him plead guilty to a lesser charge, in return for testifying against Ames. Ames is falsely found guilty of masterminding the break-in and both murders and is sentenced to death. Shelton feels betrayed by Rice's actions, as he had pleaded with him not to make the deal and to at least try convict both of them, and because he chose to release the actual killer. Darby serves his shortened sentence and is released just a few years later.

While the warden (real-life warden Frank A. Eyman) of a state prison is away, the isolation block erupts and 35 of the most violent criminals (Gene Hackman) stage a riot and take over their portion of the prison. The plot mostly involves the two main characters, Ching and Yiu and problems they face with Mickey, a fellow inmate and "Scarface", a high-ranking officer. Yiu is sentenced to three years for manslaughter. In a fight for his father, he kicked a man onto the path of a bus. Ching was sent to prison for killing his wife for being a prostitute. During the end of the film, inmates decide to go on hunger strike to protest against the cigarette price rise, this causes problems with "Assassin". Ching bit "Assassin"'s ear off and gets an extended sentence, Yiu is released from the prison.

A career bank robber, Jack Foley (George Clooney), and a U.S. Marshal, Karen Sisco (Jennifer Lopez), are forced to share a car trunk during Foley's escape from a Florida prison.
