The story is loosely based on a 1926 short story by Jun'ichirō Tanizaki, published in Chūōkōron in September 1926. The film opens as an artist and a young woman are in a dentist's waiting room. Though he is attracted to the woman, he says nothing to her. They are later in the same examining room. When the artist is given an anaesthetic, he begins to imagine a series of scenes in which the woman undergoes various forms of sexual abuse at the hand of the dentist, including rape and torture. When the artist recovers from the anaesthetic, he finds bite marks on the woman's breast, indicating that he may not have been hallucinating.

The film is a biography of anarchist Sakae Ōsugi, who was assassinated by the Japanese military in 1923. The story tells of his relationship with three women: Hori Yasuko, his wife; Noe Itō, his third lover, who was to die with him; and his jealous, second lover, Masaoka Itsuko, a militant feminist who attempts to kill him in a tea house in 1916. Parallel to the telling of Ōsugi’s life, two students (Eiko and Wada) do research on the political theories and ideas of free love that he upheld. Some of the characters from the past and from the present meet and engage the themes of the movie.

In 1980, Zaldívar was a 13-year-old who had grown up as a loyalist of the Cuban Revolution jeering in the streets at the thousands of "Marielitos" leaving the island by boat for the United States. However, within weeks, he had become a Marielito himself, headed with the rest of his family for a new life in Miami. Now a U.S.-based filmmaker, Zaldívar recounts the strange twist of fate that took him across one of the world's most treacherous stretches of water in 90 Miles.

The film follows the trials and tribulations of Eddie and other transvestites in Tokyo. The title is a pun, "rose"/"Bara" in Japanese is similar to the use of the word "pansy" in American slang.
