Sai Moon-Kin (Tsui) is a rich man who lives with his concubines and his son and a daughter named Yiau (Loletta Lee), who is raised like a boy and is ignorant of the ways of love. Because her brother is mentally retarded, Yiau wants to be her father's heir but feels that she needs to go to school for the required education. Her desire to attend school leads her to a love affair with a lustful young scholar (Ken Lok). Only after she is equipped by a protective chastity belt, does her father let Yiau go to school, guised in drag.
The mother of Nikki DeMarco (Kim Delaney) makes a pact with Satan to save her daughter's life after she has fallen from a building and nearly died. While a priest is performing the last rites, Lucifer appears, and says that he will spare her daughter's life if she promises that one day Nikki will bear his son, the Antichrist. Her mother agrees and the Devil saves Nikki. Years later, Nikki, now a successful photographer, is in a hospital watching over her mother who is now on her deathbed. Her mother tries to warn her of the impending evil, and dies. When Nikki touches her mother's crucifix her hand is burned. Nikki moves into a new apartment, not knowing that the room she is currently living in was the scene of a brutal murder years before. Nikki is skeptical of faith and religion and has become more and more isolated from spiritual ways of life.
The film opens on a train conducted by Lucifer carrying condemned souls back to Heaven. As they near, the damned begin to involuntarily sing hymns and Ms. Merrywood finds a horseshoe-shaped pendant (Shovel and Bone). In Heaven, God discusses the crisis with his favorite angel, The Agent, as Ms. Merrywood is interrogated by the sadistic Translators about her presence and the pendant. In Hell, Ticket Keeper comes to Lucifer with his misgivings about the carnies' lack of discipline and Lucifer's reticence to discuss his plans, but Lucifer dismisses him on the entrance of a cloaked figure. He opens a book entitled "The Filly and the Lapdog" (patterned on Aesop's fable "The Ass and the Lapdog") and begins to read.
In the "cartoon dimension" mad scientist Doctor Carnage experiments on a man, then disembowels him and rips his skull out through his stomach. On Earth, sisters Cindy and Candy are left alone with Cindy's friend Amy when the girls' parents go to a wedding. While Cindy and Amy call over Rick and Eddie, Candy watches a Terror Toons DVD she received in the mail up in her room. Created by Satan, the DVD depicts the antics of Doctor Carnage and his accomplice Max Assassin, a stolen lab monkey mutated into a monster by Carnage.
Douglas 'Dougie' Whooly is a nine year old boy obsessed with a video game, in which he plays Satan's little helper. His sister Jenna, who is portrayed as a pretentious actor who often speaks in a faux British accent, comes home from college for Halloween and things turn sour when Dougie finds out she brought her boyfriend, Alex, with her.

The film opens with Karen (Ilona Elkin), a traumatised woman, having a horrific nightmare involving a subway train. The flashback narrative plot follows her trapped in a subway. A Christian doomsday cult, which has been consuming and distributing hallucinogen-laced muffins that make people see visions of demons. On a texted signal, they take over services and begins massacring non-believers throughout the city, believing it is their mission to "save" the souls of humanity for God, which can only be accomplished by killing people with swords and daggers. A group of surviving train passengers and subway workers try to fight off and escape the cultists, but die one by one, leaving only Karen and two other people alive when the cultists are signaled to commit a mass suicide.
Scarlet and Chad are a young married couple. Financial troubles and the death of their infant son convince them to move to Seattle, in the hopes of starting a new life. During the drive, Chad falls asleep at the wheel and runs off the road, crashing their vehicle. Although the car itself is destroyed the couple is unharmed. They go to a nearby farmhouse and meet the owner, "Sam" (short for Samael), his wife Lilith, and their deaf handyman helper, Alal. Sam and Lilith let Chad and Scarlet sleep at their house, but during the night Chad and Scarlet are savagely knocked unconscious by Lilith and Sam.

He attempts to help save her life, but she dies at the Mercy Hospital, as a near-death patient, Caroline Kuntz (JJ Neward), awakes, kills two people and leaves. Marcus finds his life spiraling down when he witnesses his wife, Carla (Ava Gaudet), sleep with a stranger. Along with finding out Emma had died before he aided her revival, Marcus is also suspended from work for the medical mistreatment and death of a hooker, Elizabeth Markham, as Private detective Carruthers (Ving Rhames) is assigned to investigate the case.
