On Thanksgiving Day, 1983, student Marty Pascal (Josh Hamilton) brings his fiancée, Lesly (Tori Spelling), home to his family's McLean, Virginia estate to meet them for the first time. Marty is obviously nervous and hesitant about the impending introduction of his future wife to his family; he has informed them that he is bringing a guest but without any further details.

Nachiappan (Nassar) is a restaurateur in Los Angeles, USA. His twin sons Vishwanathan (Visu) and Ramamoorthy (Ramu) (both played by Prashanth), are medical students, who help him out in the evenings and alongside with the restaurant's chief cook, Juno (Senthil) who enjoys on weekends.One evening, Vishwanathan goes to the airport to check on the family's supply concession and sees that some fellow Indians – Madhumitha (Aishwarya Rai), her brother Madhesh (Raju Sundaram) and their grandmother Krishnaveny (Lakshmi) – have difficulty with an address. He pitches in to help and learns that they just flew in from India so that Krishnaveny can undergo a crucial surgery to remove her brain tumour.
The film opens with Bill Kincaid (Edward Norton) lecturing his class at Brown University about Plato's Socratic dialogues. Meanwhile, Bill's identical twin brother Brady Kincaid (also played by Edward Norton) is living in Oklahoma, growing and selling hydroponic marijuana. Brady is under pressure from local drug lord Pug Rothbaum (Richard Dreyfuss) to expand his sales. Despite needing money to repay a debt to Rothbaum, Brady refuses.
The movie centers upon the brother and sister pair Tony and Tia, whose surname they initially know only as that of their deceased adoptive parents, Malone. The children are placed in an orphanage, where they face difficulties stemming from their strange psychic/psionic abilities: Tony can psychokinetically move and control inanimate objects with the aid of his harmonica, while Tia can communicate telepathically to Tony, commune empathically with animals, and experience premonitions. Tia also possesses minor telekinetic abilities. She carries a "star case" with her at all times, which eventually reveals a strange map.

Duane Bradley arrives in New York City with a locked basket. After he gets a room at a cheap hotel, the contents of the basket are finally revealed: in it lives his deformed Siamese twin brother, Belial. Although conjoined at birth, the twins were surgically separated at an early age against their will, and Belial deeply resents being cut off from his normal-looking brother. As the twins seek revenge against the doctors responsible for their separation, Duane befriends a nurse, Sharon. Jealous, Belial attacks and kills her when he becomes frustrated with his inability to rape her. Enraged at his brother for his actions, Duane attempts to kill Belial, which results in the two brothers falling from a hotel window.

The financially troubled businessman Matthias Duval gets to know two beautiful unwed sisters. Betty and Liz Kerner are twins and they are about to inherit a fortune. But the according last will states they mustn't receive the assets unless they are married. Duval tries to solve the issue by pretending he was a twin too. He invents a twin brother and in order to make everybody believe there was a second person with his looks he creates for his imagined brother a character very different from his own. But this leads to unforeseen complications.
