In 2004, twenty-six-year-old Kentaro Oishi is repeatedly failing the national bar examination and is uncertain about his future. One day, after the funeral of his grandmother, Matsuno, he is startled to learn from his mother and older sister Keiko that his maternal grandfather Kenichiro was not his blood-relation. Keiko and Kentaro start hearing stories about their real grandfather, Kyuzo Miyabe and visit many of his former comrades all of whom begin by criticize his "timidity" in battle. Kentaro finally learns the reason why Miyabe became a Kamikaze pilot during the conversation with an old comrade of his grandfather called Izaki, who is in hospital dying of cancer. Izaki talks about his relationship with their grandfather to Keiko and Kentaro, claiming that only the "timid" Miyabe gave him the hope to save his own life after he was shot down over the ocean.

When inventor Mariko Tanaka (Hiroko Yakushimaru), who works for Hitachi appliances, accidentally re-engineers a washing machine for time travel, the Japanese government then convinces her to go back in time to prevent the passage of a fictitious law that would prevent the usage of real estate as collateral for loans, which was a major cause of the bursting of the Japanese asset price bubble. However, when contact is lost after she goes back in time, the government then sends Isao Shimokawaji (Hiroshi Abe), who works for the Finance Ministry's Emergency Response Bureau to convince Mariko's estranged daughter Mayumi (Ryoko Hirosue), to go back in time to investigate (other ministry officials tried to use the time machine but all they ended up with were faded socks).

The film begins by showing the inevitable outcome of the events of the film: the viewer is shown a scene of a man who is disabled by Alzheimer's Disease, set in the year 2010. The story proper then begins by switching back to an earlier stage in the life of the man, Masayuki Saeki in 2004. Masayuki Saeki is a brilliant and successful advertising company executive. Saeki is shown to be a prime example of an ideal Japanese white collar worker. He is strict, well organized, hard working, devoted to his job, and sets very high standards for himself and his subordinates. However he is soon shocked to realize that he is failing to meet up to his perfect standards. He starts inexplicably forgetting things - appointments, details of his work, and his knowledge of the layout of Tokyo. Following this he is diagnosed with Alzheimer's disease, to which he reacts with great anger, disbelief and despair. What follows through the rest of the film is a tragic, emotional and very human portrayal of the suffering and the decline of this once powerful, soaring man to that of a pitiful state that resembles a second childhood as the disease wears him down. As the years pass, his memory worsens. He leaves work, and lives at home, where he is cared for by his devoted wife, Emiko. Inevitably, tensions surface between Masayuki and his wife and daughter, and it reaches the point where Emiko's life revolves around taking care of her debilitated husband.

Set a year after the original, One Missed Call 2 introduces Kyoko Okudera and her friend Madoka Uchiyama. Both women are teaching assistants at a kindergarten in the middle of Tokyo. Madoka persuades Kyoko to visit a restaurant where Kyoko's boyfriend Naoto Sakurai works part-time at. Mei-Feng is the daughter of the cook, and her cellphone rings with the "ringtone of death". However, she is out buying groceries and her father answers it. The call was meant to be for Mei-Feung, but since her father answered it, he hears his own death instead. Later, when the restaurant is closed, Mei-Feung gives Kyoko and Madoka her new phone number since she has gotten a new one along with a new cellphone. Seconds after exchanging numbers, Madoka gets a call with the ringtone of death.

Manabu Yazaki had left his family's home in Hokkaidō years before to go to Tokyo to earn a degree, start a company and get married.
