Sophie (Amanda Seyfried) is a young American woman who works for The New Yorker as a fact checker. She goes on a pre-honeymoon with her chef fiancé Victor (Gael García Bernal) to Verona, Italy. Victor is unmoved by the romance of Italy and uses his time to research his soon-to-open restaurant, often neglecting Sophie. Sophie accidentally discovers an unanswered "letter to Juliet" by a Claire Smith from 1957, one of thousands of missives left at the fictional lover's Verona courtyard that are typically answered by the "secretaries of Juliet". She answers it and within a week the now elderly Claire (Vanessa Redgrave) arrives in Verona with her handsome barrister grandson Charlie (Christopher Egan). Claire and Sophie take an instant liking to each other, but Charlie and Sophie do not get along.

In 1983, Hector Negron, an aged Puerto Rican World War II veteran, works as a post office clerk in New York City. After recognizing a customer, Negron shoots and kills the man with a German Luger pistol. Several hours later, reporter Tim Boyle and Detective Tony Ricci are at the crime scene seeking information. At Negron's apartment, Boyle, Ricci and other officers discover a finely carved statue head, the Head of the Primavera, a long missing segment from the Ponte Santa Trinita. After an armored car leaves the bank with ten-million dollars, it is attacked on route by Diabolik (John Phillip Law) who manages to steal the money and escape with his partner Eva Kant (Marisa Mell). Leaving the money in their underground hideout, Diabolik and Eva attend a press conference held by the Minister of the Interior which they disrupt by releasing laughing gas. Due to the high level of crime, the death penalty is brought back. The police cannot find Diabolik but gang leader Valmont (Adolfo Celi) suffers at their hands because of a clampdown due to Diabolik's actions. Realising things can get only worse because of Diabolik's crime spree, Valmont contacts Inspector Ginco (Michel Piccoli) and makes a bargain to catch Diabolik alive for the police. In his hideout, Diabolik decides to steal the famous Aksand emerald necklace for Eva's birthday from the Saint Just Castle. Valmont builds up an identikit picture of Eva and circulates it as a means of capturing Diabolik. Diabolik scales the walls of Saint Just Castle where he finds the police are waiting, Diabolik manages to steal the necklace by fooling the police officers with mirrors on the road and dummy decoys of himself.
