In the coastal town of Astoria, Oregon approximately six months after the events of the first movie, a teenage boy named Jake asks a classmate named Emily over to his house, under the guise of studying together; however, his motives are actually centered on having her watch Samara Morgan's cursed video tape, as his seven-day deadline is fast approaching. Emily plays the tape while Jake waits in the kitchen, but when he returns to the living room, it is revealed that she covered her eyes throughout the tape, and thus didn't see any of it. As she failed to perpetuate the guidelines of the tape's curse, Samara crawls out of the television and kills Jake in front of Emily.

Some time after the events of The Ring, Samara Morgan's videotape has spread, as each person who sees the video makes a copy and shows it to someone else. A subculture has grown surrounding the video: people wait to see how close to the seven-day deadline they can get. When they grow too afraid to go on any longer, they show the tape to the next assigned person. During the interval, some create videos documenting their experiences to be posted to websites devoted to the videotape phenomenon. Groups that have watched the video are called "rings".

Fourteen year old Elizabeth Smart is part of a large and loving Mormon family. Her father, Ed hires a handyman, a self-styled prophet named Emmanuel, to help him with a remodeling job. Months later, Emmanuel returns and kidnaps Elizabeth one night at knifepoint. Her sister, Mary Katherine, who is in the bed with her, is too terrified to immediately tell her parents. Once she does a massive police investigation begins. The Smarts are angered when the investigation questions whether any family members are involved in Elizabeth’s disappearance. A massive search effort begins and the story causes a media frenzy. After a false news story that the police suspect a family member, Ed takes and passes a polygraph test. Richard Ricci, who worked for the Smarts until he was fired when jewelry went missing, is arrested for a parole violation and the police think suspect his involvement in Elizabeth's disappearance. Emmanuel subsequently tries to kidnap one of Elizabeth's cousins, but is unsuccessful. The Smarts learn Ricci has had an aneurysm and is now brain dead. After this the Mom gives up hope, the story fades into the background and the police stop looking for Elizabeth. At one point, Mary Katherine realizes that it was Emmanuel who took Elizabeth, but the police do not pursue this lead. Ed appears on the TV show America's Most Wanted and tells host John Walsh in confidence about this new information. When Walsh reveals it on Larry King Live, the Smarts publish a sketch of Emmanuel, who is later identified as Brian David Mitchell. After having taken her to San Diego, Mitchell returns to Salt Lake City with his wife and Elizabeth. When the police accidentally come across the three of them and question them, Elizabeth identifies herself, resulting in Mitchell's arrest and Elizabeth's reunion with her family.

A bomb detonates in the plaza of the Colombian Consulate building in Los Angeles, killing nine people, including a caravan of Colombian officials and American intelligence agents. Among the civilians killed are the wife and son of LAFD firefighter, Captain Gordon "Gordy" Brewer, who was injured in the explosion. A tape is sent to the U.S. State Department, in which a masked man calling himself "El Lobo" (The Wolf) claims responsibility, explaining it was in retaliation for the oppression of Colombia by the United States. The FBI believes El Lobo is a Colombian terrorist named Claudio Perrini. CIA Special Agent Peter Brandt, the Colombia Station Chief, is harshly reprimanded for the incident by a Senate Oversight Committee, who promptly terminate all CIA operations in Colombia. Brandt angrily returns to Mompós and meets with his paramilitary allies to plan a major offensive to take down Claudio.

Private detective and former Drug Enforcement agent Parrish (Lindsay Frost) is hired to be a bodyguard for a week to 8-year-old Kimberly Howard (Deborah Ann Gorman), who currently serves the key role in the custody battle of her parents James (Joseph Hacker) and Glenna (Annabella Price). Detectives Roger Mortis (Treat Williams) and Doug Bigelow (Joe Piscopo) are called to the scene of a rather violent jewelry store robbery. The robbers take on a squadron of police in a messy shootout, but neither seem affected when they are riddled with bullets. Thanks to the combined, albeit extreme measures of Mortis and Bigelow, they are able to take out the criminals, their acts narrowly avoiding termination. Meanwhile, a coroner friend of Roger's, Rebecca (Clare Kirkconnell) informs the detectives that the two bodies they had brought in had previously been to the morgue: not only do they have autopsy scars, but she herself clearly remembers performing the autopsy and has pictures to prove it, suggesting they simply got up and left the morgue at their own volition. There is a preservative chemical compound in their bodies that connect the pair to a company that had ordered a great amount of it recently. Mortis and Bigelow investigate and meet the company's head public relations person, Randi James (Lindsay Frost) who gives them a tour of the facility. When Doug wanders off to investigate a suspicious room, he encounters the reanimated corpse of a biker on a strange machine and in the fray, Roger is knocked into a decompression room used to humanely kill failed test animals and is asphyxiated to death.
