Nami Matsushima (Meiko Kaji) is set up by her boyfriend, a crooked police detective named Sugimi (Isao Natsuyagi) to win favor with the Yakuza. She is raped by several drug dealers. She makes a failed attempt to stab Sugimi on the steps of the Tokyo Metropolitan Police Headquarters. She is sentenced to do hard time in a women's prison. Matsushima is given the number 701. The prison is run by sadistic and lecherous male guards. The prisoners are forced to walk up and down a stair-like contraption naked with male guards watching from below. While in prison she meets inmates like Yuki Kida (Yayoi Watanabe) who was committed for fraud and theft, Otsuka (Akemi Negishi), jailed for burglary and extortion, and Katagiri (Rie Yokoyama) who has been impounded for arson and illegally disposing of a body. On the outside, Sugimi and the Japanese mafia orchestrate a plan in which Matsushima will succumb to an "accidental" death in prison. They enlist the help of Katagiri, pulling on her ties to both Sugimi and the mafia, and quickly set their plan in motion.

During a particularly intense Florida heatwave, inept lawyer Ned Racine (William Hurt) begins an affair with Matty (Kathleen Turner), the wife of wealthy businessman Edmund Walker (Richard Crenna). They go to great lengths to keep their affair a secret, but Ned carelessly propositions an old school friend of Matty, Mary Ann Simpson (Kim Zimmer), thinking she was Matty. Matty soon makes it clear to Ned that she wants to leave Edmund but also wants his money, explaining that a divorce would leave her with very little due to their prenuptial agreement. Racine suggests the only option is to kill Edmund. While planning the murder, Ned consults one of his shadier clients, Teddy Lewis (Mickey Rourke), an expert on incendiary devices, who supplies him with a bomb.

The film opens in New York City, where Bridget works as a telemarketing manager and her husband Clay is training to be a doctor. He is heavily in debt to a loan shark so he arranges to sell stolen pharmaceutical cocaine to two drug dealers. The transaction becomes tense when the buyers pull a gun, but rather to Clay's surprise, they eventually pay him $700,000. Gerard Reve (Jeroen Krabbé), an alcoholic, bisexual novelist, leaves Amsterdam to deliver a lecture at the Vlissingen Literary Society. The Virgin Mary appears to him in visions to show that he is targeted as her fourth victim. Mary says, "Anyone given a warning must listen to it." Gerard listens and his life is spared. He passes on the warning to Herman, Christine's other lover, who ignores it, thinking that Gerard is trying to scare him off so that he can have Christine for himself. The movie ends with Herman's death, Christine's selection of a fifth victim and Gerard's future uncertain.
