Justin (Freddie Highmore) lives in the kingdom of Gabylonia, where bureaucrats rule and knights have been ousted. In 1959, Alfred Hitchcock opens his latest film, North by Northwest, to considerable success, but is troubled by a reporter's insinuation that it is time to retire. Seeking to reclaim the artistic daring of his youth, Hitchcock turns down film proposals like adapting Casino Royale in favor of a horror novel called Psycho by Robert Bloch, which is based on the crimes of murderer Ed Gein. Gein appears in sequences throughout the film in which he seems to prompt Hitchcock's imagination regarding the Psycho story, or act as some function of Hitchcock's subconscious mind (for instance, drawing Hitchcock's attention to sand on his bathroom floor, the quantity of which reveals how much time his wife Alma has been spending at the beachhouse with Whitfield Cook).

The first third of the film is told from the main character's, Jean-Dominique Bauby (Mathieu Amalric), or Jean-Do as his friends call him, first person perspective. The film opens as Bauby wakes from his three-week coma in a hospital in Berck-sur-Mer, France. After an initial rather over-optimistic analysis from one doctor, a neurologist explains that he has locked-in syndrome, an extremely rare condition in which the patient is almost completely physically paralyzed, but remains mentally normal. At first, the viewer primarily hears Bauby's "thoughts" (he thinks he is speaking but no one hears him), which are inaccessible to the other characters (who are seen through his one functioning eye).

In 1815 Edmond Dantès, second mate of a French trading ship, and his friend Fernand Mondego, a representative of the shipping company, head to the isle of Elba to seek medical attention for their ailing captain. Dantès and Mondego are chased by British Dragoons who believe they are spies for the exiled Napoleon Bonaparte. Bonaparte himself comes to their aid, stating that they are not his agents. As payment for using his physician, Bonaparte asks Dantès to deliver a letter to a certain Monsieur Clarion in France. The captain dies, however, and they leave Elba. On returning to France, Dantès is reprimanded by the ship's first mate, Danglars, for disobeying orders. However, the company's boss, Morrell, commends Dantès' bravery, promoting him to captain over Danglars, who is left fuming. Mondego intercepts Dantès' fiancée, Mercédès, and tries to seduce her. When he hears of Dantès' promotion, Mondego realizes that Dantès and Mercédès would soon be married.
