Francesco Dellamorte (Rupert Everett) is the cemetery caretaker in the small Italian town of Buffalora. Following a 70s set prologue, the film takes place in present day Frankfurt, Germany. In medieval Germany, the Teutonic Knights massacre a village of supposed "witches" and build the titular structure over their dead bodies. In the present day, the church's new librarian (Tomas Arana) breaks the seal of the crypt out of curiosity and releases the evil spirits contained beneath it. At the same time, the church's automated mechanisms, which were set up by the architect who built the church, are triggered, causing the doors to close and trapping everyone inside, including a group of visitors. Father Gus (Hugh Quarshie), the only person not affected by the demons, eventually finds the way to make the church collapse on itself but dies in the process of doing so. Before the movie ends, Lotte (Asia Argento), the sacristan's daughter and the sole survivor of the incident, is seen walking towards the ruins of the church. She finds the seal of the crypt, opens it and peers inside. Blue light emits from within, just like when the librarian first opened it, and she smiles.

The film centers around young, insecure opera singer Betty (Cristina Marsillach). After the lead in Verdi's Macbeth is injured in a car accident, Betty is reluctantly thrust into the role in the opera. During her first performance, a murder takes place in one of the opera boxes. Mysterious murders continue throughout the film as Betty is stalked and those around her meet their unfortunate end. The killer binds and places tape under Betty's eyelids with needles attached so she is unable to blink, and therefore forced to watch as the murders take place. Meanwhile, Betty continues to have frightening dreams involving a masked person and her mother. During the final performance of the opera the killer is revealed, and Betty must confront her past in a terrifying climax.

The film takes place in a theater late at night as a troupe of actors are rehearsing a musical about a mass murderer known as the Night Owl. When the show's leading lady, Alicia (Cupisti), sprains her ankle, she and the wardrobe mistress, Betty (Schwerk), sneaks out of the rehearsal to go to a doctor, in which the closest medical facility is a mental hospital. The psychiatrist on duty is happy to help, but the wardrobe mistress notices a restrained patient on a bed lying in a room behind bars. The doctor tells them that it is Irving Wallace (Clain Parker), a former actor who went insane and murdered dozen of people. Unbeknownst to any of them, Wallace killed one of the attendants with a syringe and sneaked out of the asylum, hiding in the back of Betty's car.

After missing the bus in the Swiss countryside, a tourist, Vera Brandt (Fiore Argento), tries looking for help. Witnessing her apparent interaction with his insects, McGregor comes to believe she has a special gift for telepathy with them. Inspector Rudolf Geiger (Patrick Bauchau) is also in the case alongside McGregor. Back at the academy, the headmistress has Jennifer medically tested via EEG for her sleepwalking. The procedure makes Jennifer uneasy when she gets brief visions of the previous night's events. The headmistress then has Sophie look after Jennifer in case she sleepwalks again.
