Seven years after the events in the first film, Luc Deveraux (Van Damme), now an ordinary human, is a technical expert who is working with the government with his partner Maggie (Kiana Tom), who has been through countless hours of combat training with him, in order to refine and perfect the UniSol program in an effort to make a new, stronger breed of soldier that is more sophisticated and intelligent to reduce the use of normal, human soldiers in the battlefield. All of the new UniSols, which are faster and stronger than the original UniSols, are connected through an artificially intelligent computer system called S.E.T.H. John (Scott Adkins) awakens from a coma to find that his wife and daughter were brutally murdered in a home invasion. Haunted by images of the attack, he vows to kill the man responsible: Luc Deveraux (Jean-Claude Van Damme), one of the original Universal Soldiers (UniSol). While John tries to piece his reality back together, things are complicated by a relentless UniSol named Magnus (Andrei "The Pit Bull" Arlovski), who is determined to kill John. Meanwhile, Deveraux and surviving UniSol Andrew Scott (Dolph Lundgren) are preparing to unleash anarchy and build a new order ruled by Unisols. They are weeding out the weak and constantly testing their strongest warriors in brutal, life-and-death combat. Luc has emerged operating the Unisol Church of Eventualism, taking in wayward Unisols whom the government has been secretly operating as remote controlled sleeper agents. His mission is to liberate these Unisols from the implanted memories and the lies the government has inserted in them. As John gets closer to Deveraux and the rogue army of genetically enhanced warriors, he discovers more about himself and begins to call into question everything he believed to be true.

A group of terrorists led by Commander Topov (Zahary Baharov) kidnap the Ukrainian prime minister's son and daughter and hold them hostage, demanding the release of their imprisoned comrades within 72 hours. In addition, they have taken over the crippled Chernobyl Nuclear Power Plant and threaten to detonate it if their demands are not met. It is revealed that among the ranks of the terrorists is an experimental Next-Generation UniSol (NGU) (Andrei "The Pit Bull" Arlovski), who was smuggled in by rogue scientist Dr. Colin (Kerry Shale). U.S. forces join up with the Ukrainian army at the plant, but quickly retreat when the NGU slaughters most of them effortlessly. Dr. Porter (Garry Cooper), Dr. Colin's former colleague on the Universal Soldier program, revives four UniSols to take down the NGU, but they are systematically eliminated.

The pair experiment on students in the school basement hoping to discover the secret of reanimating corpses (akin to the work of Victor Frankenstein). Their hopes are answered when they discover a solution of Monami's blood holds the properties to bring life to dead body parts and inanimate objects.

By the 2060s, robotics technology has become extremely advanced, including cybernetics. World opinion begins to turn against robotics, leading to the U.N. declaring a unilateral ban on further research in 2067. Japan, being home to robotics pioneer Daiwa Heavy Industries, strongly protests this ban, but is unable to prevent its passage. In protest, Japan withdraws from international politics. All foreigners are deported, and further immigration is prohibited. In addition, the R.A.C.E. network is constructed — 270 off-shore installations that cover Japan with an energy field, nullifying all communication with the outside world and making satellite surveillance impossible. Trade and diplomacy continues, but Japan vanishes from the world scene.

Max Renn (Woods) is the president of CIVIC-TV, a UHF television station in Toronto that specializes in sensationalistic programming. Displeased with his station's current lineup, Max is looking for something that will break through to a new audience. One morning, he is summoned to the clandestine office of Harlan (Peter Dvorsky), who operates CIVIC-TV's pirate satellite dish which can intercept international broadcasts. Harlan shows him Videodrome, a plotless show apparently being broadcast out of Malaysia which depicts the brutal torture and murder of anonymous victims in a reddish-orange chamber. Believing this to be the future of television (seemingly staged snuff TV), Max orders Harlan to begin pirating the program.

Shekhar Subramanium (Khan), a game designer who works for the London-based Barron Industries, has delivered a number of commercial failures; an irate Barron (Tahil) gives him his last chance to develop a successful game. To impress his sceptical son Prateek (Verma), and upon the request of his wife Sonia (Kapoor), Shekhar uses his son's idea that the antagonist should be more powerful than the protagonist. His colleague, computer programmer Jenny (Goswami), uses Shekhar's face as a model for that of the game's protagonist G.One (Technically Good One and in Hindi Jeevan, which means life), while the shape-shifting antagonist Ra.One (Technically Random Access Version One and in Hindi Ravan, a mythical demon) is made faceless. Another colleague, Akashi (Wu), implements the characters' movements. The game, named Ra.One, contains three levels, the final level being the only one in which either character can be killed. Each character possesses a special device – the H.A.R.T (Hertz Amplifying Resonance Transmitter) which gives them their powers. Upon reaching the last level, the characters gain a gun with one bullet; the other character can be killed by this bullet but only if his H.A.R.T is attached.

In a small suburban town, a group of university students--Mark Loftmore (Zach Galligan), China Webster (Michelle Johnson), Sarah Brightman (Deborah Foreman), Gemma (Clare Carey), James (Eric Brown) and Tony (Dana Ashbrook)--visit a mysterious wax museum, where they encounter several morbid displays, all of which contain stock characters from the horror genre. Tony and China unintentionally enter two separate pocket worlds, as depicted by the waxwork displays, by crossing the exhibition barrier rope. Tony is at a cabin where a werewolf (John Rhys-Davies) attacks him. A hunter and his son arrive and try to kill the werewolf. The son fails and is torn in two, while the hunter shoots the werewolf, and then shoots Tony as he begins to transform into a werewolf. China is sent to a Gothic castle, where vampires attack her and Count Dracula (Miles O'Keeffe) turns her into a vampire. Two of the other students, Mark and Sarah, leave the waxwork unscathed. Later, Jonathan (Micah Grant), "a college jock", arrives at the wax museum, looking for China, but The Phantom of the Opera display gets his attention as David Lincoln (David Warner) walks him into the display. Mark goes to a pair of investigating police detectives, Inspector Roberts (Charles McCaughan), and meets Lincoln as he lets Roberts investigate the waxworks. As Mark and Roberts leave the Museum, Mark recognizes Lincoln.
